Waves OVox is pretty good, and can do it in realtime. Usually need to tweak some parameters to get clean results (and it's primarily tuned for vocals) but it's super handy.

 

Jam Origin MIDI Guitar works for me, but as the name suggests, it's guitar only.

 

This should work fine. Used it for years.

I think iZotopes one is better, vocal synth 2... seemes to be more responsive with a shitty mic (from my experiences). It also works for guitar... I put it on my buddy's guitar channel and it worked fine, a little off on the lower and really high notes but average range it worked fine

 

Afaik (and I might be wrong, haven't used it in a while) Izotope Vocalsynth doesn't output MIDI from an audio input, which is what the OP is looking for. OVox does.

Vocalsynth is amazing though. I use it as a guitar effect a lot too, you can get some crazy solo tones with it!

 

One that has not been mentioned is Vochlea Dubler 2 but it is more of an input device that transforms audio to midi than it is software even though it does have an app associated with it.. Awesome and very handy tool to have unless you can't sing, in which case it will double your workload hence the name Dubler.. LOL!

I think these two are the best. I tried several audio-to-midi plugins/apps a couple years ago. MidiGuitar 2 is freaking fast and awesome in its features. You can use it for pretty much everything.
That being said, Imitone has a couple of important options for using with voice that MidiGuitar doesn't.

Huh, the more you know. I've only used it with guitar and bass and it's fucking fantastic for that.

MIDI Guitar to me tracks better than a Roland GR/GK combo funnily enough. The only feature it lacks is really polyphonic pitchbends, though these days the "individual MIDI channels" is a chore to be honest with you. Maybe a MPE thing comes out of somewhere, that would be awesome!
