It shouldn't be too difficult for the basic amp, especially omitting the biasing output op amp and the 4 protective diodes as he mentions, it's an ultra simple and cheap design on veroboard. The problem is the +18V -18V power rails. Either you go with the multiple 9V PP3 batteries as he's done for a simple solution or use more ICs as shown for the INA217 on TIs website, which shows 3 TI ICs, a DC-DC converter and two regulators for +18V and -18V, and that's omitting providing the 48V phantom power.

If you made one I'd double check all your soldering, especially for short circuits before connecting it to anything, and I'd be really cautious before connecting it to expensive audio gear. Your $5 pre-amp may then cost you a lot more if you fry anything.
