    Headroom, headroom, headroom!

Drop all your faders to minimum. Bring your master down to between -3dB and -6dB.
Turn your monitors up a bit and bring the biggest or most important element (usually kick and sub bass) up to where you're satisfied. (some say as close to 0dB as possible, I go to -3dB to give myself plenty of headroom)
Then in descending order of importance bring each subsequent element up until it sounds good.
I take this opportunity to pan and widen certain things that I want to support the main elements. If its electronic music usually a pad or maybe the reverb tail of a synth. If it's traditionally recorded like rock, blues, jazz, etc this might be your overheads, room mics, etc. This is also where I make sure my low end is mono and I do a bit of Mid / Side processing.
But I'm not trying to get everything perfect, just get it all round about where I want. so I can start working on the tones and harmonics.

From this point I find I'm set up to start think about how I can modulate tracks to create tension and release without relying on drum rolls, sweeps and risers.

Once everything is sitting nicely together, then I can start destroying it with limiters and saturation to make it really loud and wide (assuming this is what i want).
But If everything is peaking from the beginning, you'll find it very difficult to get the best mix you can and I find aural fatigue set in really quickly making it even harder to hear subtle differences.
You should be able to hear fine adjustments at a moderate volume. If you need to pump your speakers to be able to hear the changes, chances are you need more headroom.
